Community
Health Centers (CHC) / Federally Qualified health Centers
provide comprehensive primary and preventive medical, dental,
and
mental health
care and either
(1)
receive grants under the U.S. Public Health Service Act
(i.e., Community
Health Centers, Migrant Health Programs, Health Care for
the Homeless Programs, Health Care in Public Housing Programs,
Indian Tribal Health Centers, Urban Indian Centers) or
(2)
do not receive federal PHSA grants, but meet the standards
for funding.
They
follow rigorous performance and accountability standards
regarding clinical and administrative operations. CHCs establish and utilize referral arrangements with community
hospitals and other health care providers, including specialists,
labs, therapists, and others. They also collaborate, when
appropriate, with other organizations, including hopitals,
clinics, schools,
Head
Start, WIC, Public
Health
District,
Department of Health and Welfare, etc.
They incorporate health education, community outreach, and
patient support programs in the clinical practice.
CHCs
provide services on a sliding fee scale based on household
size and income.
